I've been reading BECOMING SOMETHING, a biography of the athlete/activist/actor Canada Lee, and came across this passage on pg. 258:

Canada also got a little recording gig narrating "Jazz Band," produced by Young Peoples Records to introduce kids to jazz.  The band featured Buck Clayton on trumpet, Edmund Hall on clarinet, Teddy Wilson on piano, Sid Weiss on bass, Jimmy Crawford on drums, and Kenneth Spencer on vocals.  "In music and text, you follow the story of jazz from its birth among African slaves to its fusion with New Orleans cafe music and its travels up the Mississippi, and eventually throughout the country," noted Saul Pett, reviewing the album for the Associated Press.

This was 1948, so I'm assuming the "album" was a folio of 78s. Has this material ever been re-issued, on either LP or CD?

I have the record. It is a single 78rpm record with a printed sleeve. The term "album" is inaccurate. Not aware of any reissue.
