Hi Claude, yes I saw the same info at CliffordJordan.com. So do I understand that this may never have been issued, let alone reissued?

Not issued. Strange that it's treated as though it were.

Unless somebody comes up with a copy and it hits eBay / popsike, it's safe to assume it doesn't exist.
