has anybody ever played jazz theremin?

In his Arbors CD "Thinking Big", he uses theremin on both Ellington's "Mood Indigo" and "On a Turquoise Cloud". From the liner notes by Dan Morgenstern: -
I was going to ask if you had managed to find this Jazz Connaisseur disc for a reasonable price, but then I realized you live in Switzerland. I have paid high prices for all the Jazz Connaisseur CDs I've bought, directly from the label or from www.puppyjazz.com. Not quite reasonable, because in Switzerland in brick-and-mortar shops you easily have to pay CHF 32 to 37 for a jazz cd, unless there is a special sale (e.g. Blue Notes). This one cost me CHF 34.50. Thanks, Tommy T. Guess you'd buy cheaper from PuppyJazz or even from the Jazz Connaisseur website. And it's hard to find those CDs anywhere else!

There's a curious story behind that disc. Tete, Peer and Eric were scheduled to record a disc of Catalonian folksongs and Joan Manuel Serrat originals (which was finally called "Tete Montoliu Interpreta a Serrat"), but at the end of the evening, neither Eric nor Peer had arrived due to their engagement with another group at the Whisky Jazz club in Madrid. So Tete locked himself at the studio (Estudios Celada, Alcorcón, small town near Madrid) and played, almost without interruption, the first eight tracks. When Peer and Eric arrived, well passed midnight, they finished the disc with two traditional Catalonian songs. As they had time enough at the studio, they went into this ad lib improvisation that ended up as one of the most "out" (if that is possible with the always straight-ahead Tete) discs of his career.

Yes, it's Tete with Eric Peter on bass and Peer Wyboris on drums. The disc is called "Lliure Jazz" ("Free Jazz" in Catalan), originally recorded for Discophon and reissued on CD by Discmedi.
